The Quality Care Webinar Series is for providers seeking additional guidance and support to the HFSA Provider Risk Assessment Toolkits or interested in understanding patient risk, prevention, early detection, and treatment and management of heart failure.

              

About the HFSA Risk Assessment Toolkits:

The Health Care Provider & Patient Toolkit has been developed to allow easy and quick access to prevention management tools, to promote the HF guidelines with simple and clear messages and ultimately, to raise awareness on Heart failure risk management.

 

    • Provide guidance to providers offering clinical preventive care, health promotion, and disease management services on ways to use HF health risk assessment tools followed by evidence-based health improvement programs.
    • Ultimately, reduce health disparities through the use of HF health risk assessment tools and follow-up interventions that are linguistically and culturally tailored and are available to persons with disabilities and other underserved populations.
    • Improve health outcomes by identifying patients’ modifiable health risks and providing follow-up behavior change interventions.
