Session Description
Diabetes is a risk factor for incident heart failure and increases the risk of morbidity and mortality in patients with established disease. However, therapeutics targeting glycemic control do not necessarily reduce HF, and in some circumstances may actually increase its risk. The prevention of HF in diabetes as well as the treatment of diabetes in patients with HF represent a major challenge. The beneficial effects of SGLT2 inhibitors in HF represent an opportunity for clinicians to improve glycemic control, but also reduce HF related outcomes and to possibly improve cardiorespiratory fitness. This Session will discuss the intersection of diabetes and HF outcomes, and the role of antihyperglycemic therapies in HF with a focus on SGLT2 inhibitors and GLP1 receptor agonists and renal considerations in treatment of Hf and diabetes. Finally, a practical approach discussing the role of the endocrinologist and the cardiologist in prescribing antihyperglycemic therapies based on recent guidelines will be described.
Learning Objectives

As a result of this educational session, the learner will be able to:

  1. Recognize the role of diabetes mellitus in cardiovascular outcomes with a focus on heart failure.
  2. Outline mechanisms, potential benefits and harm of antihyperglycemic therapies in HF with a focus on SGLT2 inhibitors and GLP1RA.
  3. Debate the potential effects of antihyperglycemic therapies on cardiorespiratory fitness in heart failure.
  4. Discuss renal considerations in the treatment of diabetes and heart failure.
  5. Identify potential roles of the endocrinologist and heart failure specialist in the treatment of HF patients with diabetes incorporating recent guideline recommendations for the management of diabetes.
