Target Audience and Goal Statement

This educational activity is intended for an audience of cardiologists, including heart failure (HF) specialists, HF nurses, and nurse practitioners.

The goal of this activity is to improve clinician competence and interprofessional coordination in identifying early congestion, selecting appropriate outpatient decongestion strategies (including emerging subcutaneous [SC] approaches and remote monitoring), and operationalizing proactive care pathways to reduce hospitalizations.

Upon completion of this activity, participants will be able to:

Have increased knowledge regarding the Clinic-based vs home-based initiation of SC diuretic therapy using patient-centered and system-level considerations

Have greater competence related to:
  • Recognition of early physiologic indicators of decompensation that precede overt HF symptoms
  • Application of evidence-based strategies for outpatient decongestion for patients with and without chronic kidney disease (CKD)
  • Integration of wearable and remote monitoring data into real-world clinical workflows to trigger timely intervention
