Advanced Heart Failure & Transplant Cardiology (AHFTC) Resources

Welcome to the Advanced Heart Failure & Transplant Cardiology (AHFTC) Resources -- a collaborative space within the HFSA Learning Center designed exclusively for clinicians dedicated to the care of patients with advanced heart failure.

This platform provides AHFTC specialists, cardiologists, APPs, nurses, pharmacists, and multidisciplinary care team members with an opportunity to connect, share insights, and engage in peer-to-peer discussion around real-world challenges and emerging practices in advanced heart failure, mechanical circulatory support, and cardiac transplantation.

Participants are encouraged to:

  • Discuss complex cases and clinical considerations

  • Share best practices, protocols, and care pathways

  • Exchange perspectives on evolving therapies, guidelines, and innovations

  • Network with colleagues across institutions and care settings and share resources

Whether you’re seeking practical solutions, professional connection, or thought leadership in the field, the AHFTC Resources supports ongoing learning and collaboration beyond formal education.
